Alternate Alexandria Gilbert-Salvatore is Alex Gilbert in the alternate dimension in Alex and Stefan's minds created by Markos fifteen-hundred-years ago to create an illusion of true love for the doppelgängers.
Not much is known about this version of Alex except that she is human and happily married to Stefan Salvatore with three kids. Also, her parents, John and Isobel are alive and that she knows Grayson and Miranda as her aunt and uncle.

Season One[]
In The Big Uneasy, Alex begins having visions of a girl who looks like her except with wavy hair and dresses more girly everyday then her normal self, the girl is walking into Rousseau's talking with a girl that looks like Elena but with straight hair and dresses like how she dressed before she was turned into a vampire. Eventually Alex leaves Rousseau's and walks down the street where one of her pages from her notebook blows away. A handsome teenage guy picks it up for Alex, he smiles and after mild conversation and a look of love between them, the girl says her name is Alex and the guy is Stefan. Alex then wakes from the vision and it is revealed that Stefan had the same vision. Later, Alex was talking with Oliver, Eve, Arielle, and Hayley when she has another vision of the alternate reality: she and Stefan have come back from the movies and they both seem human and normal as they walk up to her house. Alex expresses that couldn't concentrate on the movie because she is so attracted to Stefan and they've just been to their first date. Stefan tells they should do it again sometime and they share a passionate kiss. The door opens to reveal Isobel who smiles at them. Alex introduces Stefan and Stefan shakes Isobel's hand calling her "Mrs. Gilbert" indicating that her and John are married. Stefan smiles, telling her that he's looking forward to their second date and then they say their goodbyes and as she walks into the house, Real Alex is snapped back to conscious reality by Arielle. Alex is suddenly in her shack in the bayou and wonders how she got there and Arielle reveals that she simply walked, but Alex doesn't remember as she has been having visions of the alternate reality Alex. Stefan and Alex then return to the Alternate Dimension and have more visions of Alternate Stefan and Alternate Stefan: it's been a couple of years and Alternate Stefan and Alex are seemingly living together and Stefan is trying to cook for their anniversary and Alex comes home to find Stefan cooking, who apparently can't cook in the Alternate Reality. Alex looks for a wine-opener in the drawers, Stefan tries to stop her, but she finds a wedding-ring box in one of the drawers and immediately smiles and tells Stefan "yes" and Stefan – who looks touched by her enthusiasm – tells her she didn't even open it yet and she says she didn't have to and they passionately make out in happiness as the scene is lit up by a bold light and they declare their love more each other and Stefan lifts Alex onto the counter. Both Stefan and Alex snaps out of their vision and Stefan then calls Alex to which she picks up. He asks if she's seen the visions and she told him she has. Stefan then says Elena too has seen them but feels like she's just observing. Elsewhere, Stefan and Alex continue to have visions: Alternate Alex and Stefan have gotten married and have three children. Stefan walks to Alex and tells her that he bought her aunt Miranda and Jenna a wine of the year and Alex tells Stefan that she talked to Jeremy and he'll be bringing a girl home that year and that Matt and Elena have gotten over the fight, which indicates that Matt lives in New Orleans in the reality and is with Elena. Stefan tells that Damon is, too and Alex expresses that she thinks that they'll both be lovely with lack of enthusiasm and Stefan tells her that she's lying. They share a moment in which Alex complains that Lily bit Jax and Stefan tells her that it's "nice". Alex tells him not encourage that and Stefan playfully tells her that everyone loves a biter and Alex threatens him playfully by saying she will bite him and he answers, "Good, I'll bite you right back." Markos and his Travelers begin to undo the spell and the scenes collide. Alternate Alex and Stefan kiss and then Alex is suddenly in Markos's house alone and then Real Alex awakens in the commotion with Marcel and Thierry causing.

- Alternate Alex can cook as opposed to real Alex, who is never able to cook.
- It is unknown if alternate Alex had any romantic relationship with Klaus Mikaelson, if he's real in that reality.
- Whether this reality was an actual one that the doppelgängers had mental access to and was destroyed or unbound by the Travelers.
